Becoming a patient:
If you are interested in becoming a patient, you can simply set up a new patient evaluation.
What can I expect from my first appointment?
Our new patient appointments are for one hour. Prior to coming in to the appointment, you will be sent a full medical questionnaire, which you will need to complete prior to the appointment. We also request that you bring in any tests or medical history that would be helpful to the physician. The physician will review your medical history, perform an exam, discuss your diagnoses, and design a treatment plan specifically for you. The physician will work with the other therapists in the office to incorporate all the treatments offered in our facility to treat your condition.
Insurance information:
Our office is an in-network provider with Palmetto G.B.A. Medicare. Our practitioners are B.W.C. certified. We accept PFFS Medicare Advantage Plans. We accept Tricare, United Healthcare and Anthem Blue Traditional (we are Tier 3 for all other Anthem products), as well as other insurances. Please contact the office for information.
